Het gaat bijvoorbeeld thuis helemaal niet goed, jongeren lopen vast op school of zien het leven soms echt even niet meer zitten. Dat kan komen doordat ze manisch-depressief zijn, een psychose, depressie, angst, autisme of borderline hebben en de problemen zo groot zijn dat ze vastlopen op school, thuis en met vrienden. Soms zitten ze zelf niet te wachten op zorg, en heeft de huisarts of het jeugdteam aan het (F)ACT-team gevraagd om bij ze langs te gaan.<\/p>\n<p><strong>Wat doet het (F)ACT-team?<\/strong><\/p>\n<p>(F)ACT staat voor (Flexibel) Assertive Community Treatement, ze helpen je bij de jongere in de buurt. Het (F)ACT-team is getraind en bedreven in het samen met de jongere zoeken naar wat de jongere kan en wat hem of haar motiveert. Samen werken ze eraan dat de jongere zich zo goed mogelijk kan ontwikkelen. Dat doet het team op de plek waar dat het hardst nodig is: thuis, op school of bij de sportclub bijvoorbeeld.<\/p>\n<p><strong>Welke behandelingen worden er gegeven?<\/strong><\/p>\n<p>Als een jongere in intensieve behandeling en begeleiding komt bij het (F)ACT-team, wordt altijd samen gekeken welke hulp het hardst nodig is. Misschien zijn dat individuele gesprekken, gezinstherapie, ouderbegeleiding voor de ouders of een groepsbehandeling. Als er ook hulp nodig is bij praktische zaken, zoals werk zoeken of schulden aflossen, dan wordt er samen gezocht welke hulp daarbij nodig is.<\/p>\n<p><strong>Wie werken er in het (F)ACT-team?<\/strong><\/p>\n<p>Het (F)ACT-team bestaat uit verschillende experts: een ervaringsdeskundige, (sociaal psychiatrisch) verpleegkundigen, sociaal pedagogisch werkers, ouderbegeleiders, gezinstherapeuten, psychologen, een kinder- en jeugdpsychiater en trajectbegeleiders.
